Registering with ADS

Employees requesting an accommodation due to a qualifying disability or health condition should self-identify by registering with the Office of Accessibility and Disability Services (ADS). This process involves the following steps:

  • Completing the ADS registration form
  • Providing ADS with current medical and/or psychological documentation (from a qualified professional who is not a relative) to verify the presence of a qualifying disability. Employees are encouraged to have their medical provider complete the ADA Accommodation Medical Information Request form. If more information is needed regarding the work being performed, position descriptions are available from Human Resources. Additional performance criteria within the PMP (staff) or FAR (faculty) processes may inform the discussion with the medical provider.
  • Once adequate documentation has been provided, the request for accommodation will be reviewed by the ADA Coordinator in terms of what is effective given the essential job functions and operational necessities, while avoiding undue hardship situations.
  • Meeting with the ADA Coordinator for a confidential review and analysis of documentation and to discuss possible workplace accommodations
  • Formal accommodation determination results in a letter describing the accommodation that informs the employee and their supervisor of the accommodation, its anticipated duration, and the time period for revisiting the process.
